SAFE WORK METHOD STATEMENT

SAFE WORK METHOD STATEMENT (SWMS)

The visitor FROM Matara B.A. will at all times:

  • Be accompanied by an Operating Company (Operator) -appointed supervisor responsible for the relevant work area;
  • Comply with all Operator site rules, inductions, and instructions;
  • Wear full personal protective equipment (PPE) in accordance with Operator's standards.

1. Open Pit Mining Areas

Scope of Activity: Observational visits to open pit mining areas including pit crests, haul roads, and designated viewing locations.

Key Hazards: Mobile mining equipment, vehicle traffic, pit wall instability, blasting activities, dust, noise, weather exposure.

Control Measures:

  • Access only under the supervision of the designated Open Pit Supervisor.
  • Compliance with site traffic management plans and exclusion zones.
  • No access to pit floors, active blast areas, or geotechnically restricted zones unless expressly authorized and accompanied.
  • Maintain safe stand-off distances from pit crests and operating equipment.
  • Mandatory PPE including high-visibility clothing, safety helmet, steel-capped boots, eye and hearing protection.

Residual Risk: Low, subject to application of the above controls.

2. Underground Mining Areas

Scope of Activity: Observational visits to underground mining areas via approved access points and travel ways.

Key Hazards: Ground instability, mobile equipment interaction, low visibility, ventilation hazards, heat, noise, confined spaces.

Control Measures:

  • Entry only with the Underground Supervisor following completion of underground induction.
  • Compliance with tag-in / tag-out, communications, and emergency response procedures.
  • Travel restricted to designated access ways and refuge areas as directed.
  • No entry to unsupported headings, active stopes, or blasting areas.
  • Mandatory PPE including cap lamp, self-rescuer, high-visibility clothing, helmet, and steel-capped boots.

Residual Risk: Low, subject to application of the above controls.

3. Tailings Storage Facilities (TSF)

Scope of Activity: Visual inspection of TSF embankments, decant structures, and associated infrastructure from approved locations.

Key Hazards: Soft or unstable ground, embankment instability, water hazards, mobile equipment, environmental exposure.

Control Measures:

  • Access only with approval and supervision of the responsible TSF Supervisor.
  • Remain on designated access tracks and approved observation points.
  • No access to ponded water areas, embankment crests, or unstable ground.
  • Compliance with site-specific TSF safety procedures and PPE requirements

Residual Risk: Low, subject to application of the above controls.

4. Processing Plant Areas

Scope of Activity: Observational visits to crushing, processing, and ancillary plant areas.

Key Hazards: Moving machinery, elevated walkways, noise, chemicals, electrical hazards, hot surfaces.

Control Measures:

  • Access only under supervision of the Processing Plant Supervisor.
  • Remain within designated walkways and exclusion zones.
  • No interaction with operating equipment or control systems.
  • Compliance with lock-out/tag-out signage and chemical safety requirements.
  • Mandatory PPE including helmet, eye protection, hearing protection, gloves, and safety footwear.

Residual Risk: Low, subject to application of the above controls.

1.1 General Conditions

  • All required site inductions must be completed prior to site access.
  • Instructions from Operator's supervisors and HSE personnel must be followed at all times.
  • Any unsafe condition observed will be immediately reported to the supervising Operator representative.
  • This SWMS applies solely to observational site visits and does not authorise operational or physical work activities.
